ফার্মাকোলজি
Rivaroxaban is a highly selective direct inhibitor of factor Xa. By blocking factor Xa it interrupts the coagulation cascade and prevents thrombin formation and clot development.
Rivaroxaban ফ্যাক্টর Xa-এর একটি অত্যন্ত নির্দিষ্ট ও সরাসরি প্রতিরোধক। ফ্যাক্টর Xa বন্ধ করে এটি রক্ত জমাট বাঁধার প্রক্রিয়া ব্যাহত করে এবং থ্রম্বিন তৈরি ও রক্ত জমাট বাঁধা প্রতিরোধ করে।
মাত্রা ও সেবনবিধি
Atrial fibrillation: 20 mg once daily with food (15 mg once daily in moderate renal impairment).
Deep vein thrombosis or pulmonary embolism: 15 mg twice daily with food for 21 days, then 20 mg once daily.
After hip or knee replacement: 10 mg once daily.
অ্যাট্রিয়াল ফিব্রিলেশন: 20 mg দিনে একবার খাবারের সাথে (কিডনির মাঝারি সমস্যায় 15 mg দিনে একবার)।
ডিপ ভেইন থ্রম্বোসিস বা পালমোনারি এম্বোলিজম: প্রথম ২১ দিন 15 mg দিনে দুইবার খাবারের সাথে, এরপর 20 mg দিনে একবার।
হিপ বা হাঁটু প্রতিস্থাপনের পর: 10 mg দিনে একবার।
পার্শ্বপ্রতিক্রিয়া
Bleeding (nosebleed, gum bleeding, bruising, blood in urine or stool), anaemia, nausea, dizziness, headache and raised liver enzymes.
রক্তক্ষরণ (নাক বা মাড়ি দিয়ে রক্ত পড়া, কালশিটে পড়া, প্রস্রাব বা মলের সাথে রক্ত), রক্তস্বল্পতা, বমি বমি ভাব, মাথা ঘোরা, মাথাব্যথা এবং লিভার এনজাইম বৃদ্ধি।
সতর্কতা
Do not stop without medical advice, as the risk of clots rises. Use with caution in renal impairment, in the elderly and with other drugs that increase bleeding. Inform the doctor before surgery or dental procedures.
চিকিৎসকের পরামর্শ ছাড়া বন্ধ করা যাবে না, কারণ এতে রক্ত জমাট বাঁধার ঝুঁকি বাড়ে। কিডনির সমস্যা, বয়স্ক রোগী এবং রক্তক্ষরণ বাড়ায় এমন ওষুধের সাথে সতর্কতা প্রয়োজন। অস্ত্রোপচার বা দাঁতের চিকিৎসার আগে চিকিৎসককে জানান।
যাদের জন্য নিষেধ
Active clinically significant bleeding, conditions with a high risk of major bleeding, liver disease with coagulopathy, prosthetic heart valves, pregnancy, breast-feeding and hypersensitivity to rivaroxaban.
সক্রিয় ও উল্লেখযোগ্য রক্তক্ষরণ, বড় ধরনের রক্তক্ষরণের উচ্চ ঝুঁকিযুক্ত অবস্থা, কোয়াগুলোপ্যাথিসহ যকৃতের রোগ, কৃত্রিম হার্ট ভালভ, গর্ভাবস্থা, স্তন্যদানকাল এবং Rivaroxaban-এর প্রতি অতিসংবেদনশীলতা।
সংরক্ষণ
Store below 30°C in a cool, dry place, away from light. Keep out of the reach of children.
৩০°সে. তাপমাত্রার নিচে, আলো থেকে দূরে, ঠান্ডা ও শুষ্ক স্থানে রাখুন। শিশুদের নাগালের বাইরে রাখুন।
