Prophylaxis of gout and of uric acid and calcium oxalate kidney stones; prophylaxis of hyperuricaemia associated with cancer chemotherapy.

ফার্মাকোলজি
Allopurinol and its active metabolite oxipurinol inhibit xanthine oxidase, the enzyme that converts hypoxanthine and xanthine to uric acid, and so lower uric acid levels in blood and urine.
Allopurinol ও এর সক্রিয় বিপাকজাত অক্সিপিউরিনল জ্যানথিন অক্সিডেজ এনজাইমকে বাধা দেয়, যা হাইপোজ্যানথিন ও জ্যানথিনকে ইউরিক অ্যাসিডে রূপান্তর করে। ফলে রক্ত ও প্রস্রাবে ইউরিক অ্যাসিডের মাত্রা কমে।
মাত্রা ও সেবনবিধি
Adult: initially 100 mg once daily after food, adjusted according to the serum uric acid level; usual maintenance 100 to 300 mg daily, up to 900 mg daily in severe cases (doses above 300 mg in divided doses).
Child under 15 years (in malignancy): 10 to 20 mg/kg daily; maximum 400 mg daily.
Reduce the dose in kidney impairment.
প্রাপ্তবয়স্ক: শুরুতে ১০০ mg দিনে ১ বার, খাবারের পর; রক্তে ইউরিক অ্যাসিডের মাত্রা অনুযায়ী সমন্বয় করতে হবে; সাধারণ মাত্রা দৈনিক ১০০ থেকে ৩০০ mg, তীব্র ক্ষেত্রে দৈনিক ৯০০ mg পর্যন্ত (৩০০ mg-এর বেশি হলে বিভক্ত মাত্রায়)।
১৫ বছরের কম বয়সী শিশু (ক্যান্সারের ক্ষেত্রে): দৈনিক ১০ থেকে ২০ mg/kg; দৈনিক সর্বোচ্চ ৪০০ mg।
কিডনির সমস্যায় মাত্রা কমাতে হবে।
পার্শ্বপ্রতিক্রিয়া
Skin rash, nausea, vomiting and raised liver enzymes. Rarely severe hypersensitivity reactions, including Stevens-Johnson syndrome, with fever and liver or kidney involvement.
ত্বকে ফুসকুড়ি, বমি বমি ভাব, বমি ও লিভার এনজাইম বেড়ে যাওয়া। কদাচিৎ স্টিভেন্স-জনসন সিনড্রোমসহ তীব্র অতিসংবেদনশীলতাজনিত প্রতিক্রিয়া, জ্বর এবং লিভার বা কিডনির ক্ষতি হতে পারে।
সতর্কতা
Stop the medicine immediately if a skin rash or other sign of allergy appears. Drink plenty of fluid (2 to 3 litres daily). Reduce the dose in kidney or liver impairment. An acute attack of gout may occur when treatment is started; an NSAID or colchicine is usually given for the first few months.
ত্বকে ফুসকুড়ি বা অ্যালার্জির অন্য কোনো লক্ষণ দেখা দিলে সাথে সাথে ওষুধ বন্ধ করতে হবে। প্রচুর পানি পান করুন (দৈনিক ২ থেকে ৩ লিটার)। কিডনি বা লিভারের সমস্যায় মাত্রা কমাতে হবে। চিকিৎসার শুরুতে গেঁটেবাতের তীব্র আক্রমণ হতে পারে; এ জন্য প্রথম কয়েক মাস সাধারণত NSAID বা Colchicine দেওয়া হয়।

Hypersensitivity to allopurinol. Not for the treatment of an acute attack of gout.

সংরক্ষণ
Store below 30°C in a cool, dry place, away from light. Keep out of the reach of children.
৩০°সে. তাপমাত্রার নিচে, আলো থেকে দূরে, ঠান্ডা ও শুষ্ক স্থানে রাখুন। শিশুদের নাগালের বাইরে রাখুন।
